NOTICE: U.S. Passport fees are being increased sharply as of Monday, July 12, 2010. As a result, the City of Takoma Park will provide Passport Agent services on Friday, July 9 from 9:30 am to 1:30 pm at the main reception desk of the Takoma Park Community Center, 7500 Maple Avenue. What are your hours of operation?

The City of Takoma Park accepts passport book/card applications from Monday through Thursday, 9 am to 1:30 pm. Please call 301-891-7100 to confirm.

What do I need to do to apply for a U.S. Passport Book?

• Complete and Submit form DS-11 available at the Municipal Building or by down loading application form at travel.state.gov.

• Submit original evidence of U.S. Citizenship

• Present Valid Photo I.D:
  • Previously issued, undamaged U.S. Passport
  • Naturalization Certificate
  • Driver's License
  • Current Government I.D (City, State, or Federal)
  • Current Military I.D (Military and Dependants)

• Submit a photocopy of the Indentification document(s) presented
  • Photocopy must be on plain white 8 1/2 X 11 standard paper stock showing the front and back of your I.D
  • Photocopy must contain images on only one-side of each paper submitted

• Pay the applicable fee
• Provide two identical, 2x2 color passport photos.

What are the passport fees?

U.S. PASSPORT BOOKS

Age 16 and Older
Total cost of $135 includes: Passport Fee of $110 (to be paid to U. S. Department of State only by personal check or money order) plus $25 Execution Fee (to be paid to the City of Takoma Park by personal check, money order or cash)

Under Age 16
Total cost of $105 includes: Passport Fee of $80 (to be paid to U. S. Department of State only by personal check or money order) plus $25 Execution Fee (to be paid to City of Takoma Park by personal check, money order or cash)

Expedited Services: Add $60 for each application made payable to the U.S. Department of State
File Search Fee: $150

NEW U.S. PASSPORT CARD
The Department of State has developed a Passport Card as a more portable and less expensive alternative to the traditional passport book. The New Passport Card can ONLY be used for land and sea travel. The fees will be $30 for adults and $15 for children, plus a processing fee of $25.00. The City of Takoma Park will accept applications for the card.

How do I renew my passport book?

You can renew your U.S. Passport Book IF ...

• you can submit your most recent passport with your application
• you were at least 16 years old when your passport was issued.
• your most recent passport was issued less than 15 years ago
• your name has not changed since your last passport was issued OR your name was changed by marriage or court order and you can submit proper documentation to reflect the name change.

then you renew your passport by completing form DS-82 and submitting your application by mail. The cost is $110; if you choose expedited service, the total fee is $170 to the U.S. Department of State.

IF YOU are NOT eligible for renewal , you must apply for a U.S. passport via the regular process described above.
